We recommend keeping a copy of this in a place known to at least two members of your family and where possible a copy should be made available to your executor, next-of-kin or funeral director.
Please Note: The completion of this document will provide your executor or legal administrator with the necessary information required when bereavement occurs. It will also alleviate the burden and stress caused to the family of the deceased. This is not a legal document. Be sure that you have made a will and it is up to date.
